Note: Your coverage may vary depending on the details of your specific plan. Once you’ve completed your intake form and booked an appointment, we’ll verify your coverage and share the results with you. If you’d like to verify your coverage before booking, simply text a photo of both sides of your insurance card to our team at 602.694.6313 or 720.856.6614.

At this time, we’re unable to accept Medicaid plans.

We wish we could serve everyone, but at this time, our practice is not in-network with Medicaid. That means we’re unable to provide services to anyone enrolled in Medicaid, even if it’s combined with another type of insurance.

Not sure if you have a plan with Medicaid?

The easiest way to determine this is to look at your insurance card to see if it includes one of the following logos or the word “Medicaid” on the card. If it does, you’re enrolled in Medicaid.

This means Health First Colorado and AHCCS plans are not accepted.

But don’t worry — you can reach out to your insurance carrier for a list of in-network providers.
